This form submission is an official request to utilize a campus facility for the requestor-sponsored event or activity. The individual(s) listed is responsible for coordinating all event/activity logistics.

As a courtesy and to ensure proper staff coverage and room availability, facility requests should be submitted two weeks before the date you requested. Requests outside of the two weeks will be considered on a case-by-case basis.

NOTE: This form is just a request and is NOT a confirmation of your room reservation. You will receive a confirmation from the appropriate campus representative on the status of your request.
